Assessments and Referrals
Referrals for Brentwood Hospital, located in Shreveport, Louisiana, can be made by anyone seeking help for themselves, a loved one or friend, a patient or client. Before a patient is admitted, they must undergo a no-cost assessment to determine the level of care necessary to stabilize the patient. From there, we ask that patients and/or guardians fill out the appropriate paperwork, which can be found here. Delays in receiving the proper forms could slow down the admissions process.
